Send your name, address, and telephone number along with your desired inscription and a check for $300.00 per brick made out to Pet Pride of New York, P.O. Box 338, Mendon, NY 14506, Attention: Marlies.

Inscriptions can be three lines of text and up to 15 characters per line. Each space between words also counts as a character.
